Evaluation of Results of Coaxial Phacoemulsification with Implantation of an Intraocular Lens through a Microincision

Summary: Purpose: In the recent years, a fast development of surgical techniques in cataract surgery can be observed. It is connected with the introduction of new technologies, phaco machines, surgical instruments and intraocular lenses. Microincision cataract surgery raised much interest recently, as it is perceived as the next step in cataract sugery. The purpose of the study was to evaluate functional results after coaxial 1.8mm microincision surgery (C-MICS) with intraocular lens implantation. Materials and Methods: The examined group consisted of 98 eyes of 98 consecutive patients operated using coaxial 1.8 mm microincision cataract surgery, with a foldable intraocular lens implantation. In the examined group, there were 62 women (63%) and 36 men (37%), in age from 45 to 92 years (mean age 73 ?10 years). We examined: objective reafraction using autorefractometry (Tomey TR 400), manual Javal keratometry, best corrected visual acuity using standard Snellen charts, and corneal endothelial cell density using confocal microscope Topcon SP 2000P. Anterior and posterior segment of the eye were also evaluated. Surgically induced astigmatism (SIA) was calculated using vector analysis and Naeser?s method. Results: There were no intraoperative and postoperative complications in any patient. Visual acuity improved postoperatively in all patients and the implanted lens was positioned centrally. Best corrected visual acuity amounted to 0.36?0.25 preoperatively and improved to 0.96 ? 0.1 postoperatively. Surgically induced astigmatism caused by C-MICS was minimal ? according to vector analysis mean SIA was 0.35 ? 0.28 D. Mean corneal endothelial cell density loss was 10.2%. Conclusions: Coaxial 1.8 mm microincision cataract surgery, with a foldable intraocular lens implantation is a safe technique and enables to achieve very good functional results and fast visual rehabilitation. Both surgically induced astigmatism and corneal endothelial cell density loss caused by C-MICS were very small.
