Psychogenic and Simulated Vision Problems in Children – Our Experience

Summary: Purpose: A retrospective analysis of clinical features of visual problems in children with psychogenic visual deterioration and simulation.
Material and methods: 10 patients with monocular or binocular decreased visual acuity were examined in Department of Ophthalmology in Children Hospital between 2014 and 2016. Basic ophthalmological examination – visual acuity, cycloplegic refraction, biomicroscopy, indirect ophthalmoscopy, optical coherence tomography of macula and optic nerve, automated and kinetic perimetry. In addition, in some cases we performed neuroimaging studies, electrophysiologic testing and consultation of other specialists. We also investigated the psychosocial situation and family and emotional problems.
Results: 7 children were diagnosed as malingering and simulation. In 3 cases clinical syndromes indicated conversion problems.
Conclusion: If basic ophthalmological examination and additional tests in children reveal no cause of vision deterioration a simulation or conversion should be considered.
