Diagnosis and Treatment of Circumscribed Choroidal Hemangioma

Summary: Circumscribed choroidal hemangiomas are hamartomas occupying the entire thickness of choroid. One characteristic symptom is the presence of an oval tumor in fundus of the eye, typically positioned temporaly from the optic disc and in its close vicinity. The edges of the circumscribed choroidal hemangiomas become visible as a result of induced pressure on the choroid and melanocytes. Choroidal hemangiomas are composed of dilated vessels separated by connective tissue. These tumors grow slowly and do not have proliferative potential. They consist of a mature and orderly tissue, which determines their slow growth comparing to other intraocular tumors. All kinds of vessels are prone to malformations. Most common are: capillary and cavernous hemangioma, or combinations of both. Circumscribed choroidal hemangiomas develop in patients regardless of their gender in the middle age group, in definite majority of cases they are situated in the posterior pole of the eyeball or in the immediate proximity of the north pole. During the first visit each patient requires angiography and optical coherence tomography or swept source optical coherence tomography. The tests have to be repeated after every completed treatment. Still, the most valuable diagnostic method that enables the differentiation of intraocular tumors is ultrasound in the presentation A and B. The decision regarding the possible treatment and its type should be based on the results of the tests, as well as the presented symptoms and should be individual for each patient. Patients who show no signs of changes related to the presence of circumscribed choroidal hemangioma should be observed. Treatment results such as decrease in tumor size and improvement in the visual acuity depend on the chosen method of treatment, the initial size of the hemangioma and the duration of symptoms before the treatment.
