Spontaneous Carotid Cavernous Direct Fistula – Case Report

Summary: Carotid cavernous fistula is an abnormal connection between a cavernous sinus and carotid arteries (internal and external). This leads to mix arterial and venous blood. The most common classification divides them into direct and indirect fistula which differ dynamics of the appearance of clinical symptoms. A triad of symptoms include: pulsating exophthalmia, swelling and redness of the eye, vascular noise, occurring mainly in direct fistulae. A case presents 51 year old woman with spontane‐ ous direct carotid-cavernous fistula who came to the Ophthalmology Clinic due to slightly intensified pulsating exophtamia of the left eye with splitting and conjunctival congestion. After general diseases exclusion and imaging diagnosis execusion, spontaneous direct fistula has been recognised, although the symptoms have suggested indirect form. In the case of this patient the fistula has had a spontaneous closure.
