Blunt Ocular Trauma with Hyphema in Children – Own Material

Summary: Purpose: To describe cases of hyphema caused by eye blunt injury in children.
Material and methods: Retrospective analysis of 27 patients with traumatic hyphema admitted to the Department of Ophthalmology of J. Bogdanowicza Children’s Hospital in Warsaw between January 2011 and December 2016, aged 2–16 years. The cause of injury, ophthalmology state at admission and discharge and treatment were considered.
Results: 12 patients (44%) had visual impairment at admission. 13 patients (48%) had visual impairment at admission. Other complications were: erosion (6 patients – 22%), corneal edema (8 patients – 30%), elevated intraocular pressure (8 patients – 30%), angle recession (19 patients – 70%), retinal or vitreous hemorrhages/vitreous effusion (9 patients – 33%), choroidal rupture (3 patients – 11%). At admission 5 children (19%) had hyphema in 0 grade, 19 children (70%) – 1 grade, 3 children (11%) – 2 grade. Rebleeding occurred in two patients (7%) with 1 and 2 grade of hyphema. Anterior chamber washout was performed in one patient.
Discussion: The traumatic hyphema was observed mostly in boys, during sports and play. It is a common consequence of blunt trauma, usually rapidly absorbs, without serious consequences. Sometimes it causes complications such as glaucoma, rebleeding, corneal blood staining or amblyopia.
Conclusions: Hyphema caused by blunt injury in children is usually mild. Due to the risk of secondary hemorrhage and other complications, hospitalization, careful observation and on-time treatment is necessary.
