Summary: Corneal decompensation is a condition caused by an accumulation of the intraocular fluid in its structure. It causes the edema of this tissue, with the most severe form called bullous keratopathy. The loss of corneal endothelial cells is the most frequent cause for development of this condition. It may be seen in the course of corneal dystrophies, e.g. Fuch’s endothelial dystrophy or after intraocular inflammations and surgical aggressive procedures. One of the relatively less known and frequently forgotten cause may be also an endotheliopathy in the course of pseudoexfoliation syndrome. The presence of this condition may lead to corneal decompensation after surgical procedures used for treatment of pseudoexfoliation syndrome complications as cataract and glaucoma. So the assessment of the posterior cornea should be always done before intraocular procedures in patients with pseudoexfoliation syndrome.
