Autologous Serum Eye Drops as a Therapeutic Option in Case of Non-healing Corneal Erosion in Patient with Diabetic Retinopathy – Case Report

Summary: Introduction: Corneal erosions in patients with diabetes present a huge problem in clinical practice. They cause a variety of complains, including decreased visual acuity and lower quality of patients’ life. Erosions are often resistant to treatment, which may result in worsening of the corneal condition leading to perforation. Apart from widely used commercial products, there is a possibility to perform therapy using autologous serum eye drops. They contain a variety of chemical compounds necessary to maintain the integrity of the cornea.
Aim: To present a case of a patient with extensive corneal erosion unresponsive to standard treatment. Case report: A 70-year-old patient with proliferative diabetic retinopathy after phako-vitrectomy of his right eye due to blood hemorrhage in to the vitreous chamber, had the follow-up visit, at which huge corneal erosion was detected. After two months of the topical and bandage contact lens therapy the effect was disappointing. The 2nd degree of neurotrophic keratopathy was diagnosed. It was decided to start the treatment with autologous serum eye drops. At the first control visit after a week of using serum drops, the erosion area was already reduced by about 50%. The patient reported the pain reduction. To support epithelialization the new bandage contact lens was used. At the next follow-up visit erosion was almost healed and visual acuity was improved. The patient remains under control of the Ophthalmology Clinic.
Conclusions: Autologous serum eye drops are used for the treatment of dry eye syndrome and non-healing corneal erosions when other methods fail. Low cost, good profile in terms of safety and effectiveness encourage to use them.
