Use of Intracorneal Rings in Keratoconus Treatment

Summary: Intacs represent a new method for improving visual level in keratoconus patients who cannot improve satisfactory visual acuity in spectacle or contact lens correction. Each ring, made of polymethyl methacrylate (PMMA), has a hexagonal cross-section, with an outer diameter of 8.1 mm and inner diameter of 6.8 mm. The rings are designed to alter the anterior corneal curvature. The degree of cornea flattening of keratoconus reduction depends on how this ring is inserted in the peripheral cornea. Intacs cause a local separation of the corneal layers, thus having a direct effect on flattening of the cornea and reducing myopia and myopic astigmatism. The thicker the rings, the greater the layer separation and flattening. The study discusses contraindications for and technique of inserting intracorneal rings based on own experience. Conclusion: Based on literature and own observation, we can say that although intracorneal rings do not eliminate progression of keratoconus growth, they can delay the necessity to transplant the cornea, slow down the the process of corneal thinning and allow the patients to return to wear contact lenses.
