Summary: Abnormalities in the ocular organ caused by diabetes mellitus have different character. In majority they are related to diabetic angiopathy. Their pathogenetic mechanisms embrace the phenomena, which can not be influenced by therapy like genetics, time course, sex and factors which could be limited by therapeutic intervention of general (diabetological) character and local (ophthalmological) approach. They are: metabolism control, blood pressure, microvessel hemodynamics, endothelial function, coagulation changes. Therapy of these general, pathogenetic disturbances contributing to the appearance of the vascular degenerative symptoms in the retina and in the eye should be programmed as the holistic, “shared” diabetological-ophthalmologic care.
