Role of the Doppler Examination in Early Stages of Age-Related Macular Degeneration

Summary: Aim: Assesment of the relationships between blood flow disturbances in the ocular vessels in patients with age-related macular degeneration and advancement stage of the macular lesions, expressed in the fluorescein angiography image and visual function.
Material and Methods: Group of 51 individuals with age-related macular degeneration was examined, including 33 women (65%) and 18 man (35%), age from 64 to 91 years (SD±4.11). Patients with typical, identified lesions in the macular area, both with subjective symptoms and without, were qualified. Complete ophthalmologic examination was performer. That examination consisted of visual acuity evaluation and anterior and posteriori segment examination, extended by fluorescein angiography. In all patients, standard parameters of the blood flow in central retinal artery and posterior short ciliary vessels using Color Doppler US, with high frequency probe of 12 MHz of the Toshiba device were performed. Differences underwent statistical analysis.
Results: Statistically significant reduction in ocular vessels blood flow was observed, and what is interesting – even in patients with initial degenerative lesions, before fully-symptomatic AMD development in the FA image. Worse flow did not significantly correlate with central visual acuity decrease.
1) Blood flow disturbances in retinal vessels already occur in initial, asymptomatic stages of age-related maculopathy, thus Color-Doppler US method may serve as an early prognostic examination of the fully-symptomatic AMD development.
2) Reduction in retinal vessels flow in individuals with initial visual acuity disturbances may suggest vascular etiology of the age-related macular degeneration.
