Comparison of Intraocular Pressure Measurement with Goldmann Applanation Tonometer and Diaton Transpalpebral Tonometer

Summary: Purpose: To compare the intraocular pressure (IOP) measured by transpalpebral Diaton tonometer and Goldmann applanation tonometer and to investigate the influence of central corneal thickness, artificial intraocular lens, trabeculectomy on the measurements.
Materials: 126 patients at the age of 30-92 (mean 71) were enrolled in the study. The study group comprised 86 women and 40 men with the diagnosis of glaucoma or ocular hypertension, Caucasians.
Methods: IOP measurements were obtained in all patients using Goldmann applantaion tonometer and transpalpebral Diaton tonometer (patient in horizontal position). Central corneal thickness (CCT) was measured by means of noncontact optical low coherence reflectometry (OLCR pachymeter) with a programme that corrects value of pressure measured with Goldmann applanation tonometer.
Results: Mean intraocular pressure measured by Goldmann applanation tonometer was 13.6 ± 2.6 mmHg (min. 6, max. 25 mmHg), while the transpalpebral tonometer: 15 ± 3.4 (min. 4, max. 23). The average difference between the measurements was 1.52 ± 3.41 mmHg (p<0,00001).
The average difference between the measurements made with transpalpebral tonometer (T-przezpowiekowo) and the value of intraocular pressure corrected with pachymeter (T-tonofit) was 2.28 ± 3.39 mmHg (p<0,00001).
There was no impact of the presence of an artificial intraocular lens on a difference in the measured values.
In the group of patients after trabeculectomy (29 eyes) the difference between the measurements was not statistically significant (p = 0.12).
Conclusions: Diaton transpalpebral tonometer is not recommended as alternative to Goldmann applanation tonometer but may be helpfull as a screening tool in IOP measurements or in cases when it is not possible to use Goldmann applanation tonometer.
