Summary: The aim of the study was the presentation of clinical and laboratory difficulties in prognosis of severe ROP stages. Although there are known risk factors of ROP development, which are mainly connected with rescuing and life sustaining of prematurely born babies, there is lack of prognostic indices for progression of active ROP forms in daily use. Thus, in the article it has been indicated that there are the possibilities of utilizing selected growth factors and vascular-derived substances, including pro-inflammatory cytokines, which could be prognostic in severe ROP. Scientific and technical advancement and the use of larger number of modern medical equipment on Intensive Care Units sustain the life of bigger amount of premature infants. Multifactorial reasons for ROP might lead to irreversible refractive errors, major vision retardation or even blindness. Fibro-vascular proliferative changes localized in posterior eye chamber which in clinical course of the disease may result in secondary pathologies such as: glaucoma, cataracts, strabismus and nystagmus. Extra-lens proliferations in ROP appear in advanced stages of the diseases and generally are associated with negative prognosis being the cause for retinal ablation. In the article the effects of surgical treatment and recent direction of application for new therapeutic strategies in ROP treatment have been depicted.
