Preservatives in Ophthalmic Medications

Summary: Preservatives are an important component of multidose ophthalmic medications, providing antimicrobial activity and preventing from decomposition of the active drug. However, the presence of preservatives in eye drops may induce ocular surface changes, causing ocular discomfort, tear film instability, conjunctival inflammation, subconjunctival fibrosis, epithelial apoptosis and corneal surface impairment. The severity of these effects is related to types and concentrations of preservatives as well as duration of their application. The most commonly used preservative in ophthalmic medication is benzalkonium chloride. Other preservatives such as poliquaternium-1, chlorobutanol, chlorhexidine acetate, sodium perborate, stabilized oxychloro complex and SofZia are also commonly used in topical drugs. In the present review all these ophthalmic preservatives, their benefits, mechanism of action and side effects were discussed.
