Summary: Aphakia is a state of the absence of the lens of the eye. It may be caused by trauma or be the intended outcome of surgery or surgical complication, especially in eyes with congenital or acquired zonular weakness. It must be remembered, that prolonged silicone oil tamponade could lead to zonular atrophy. The objective of the treatment is to achieve the most natural conditions. Most preferred is the implantation of an artificial lens in place of the missing one. The ideal is to insert the implant in the capsular bag, because it provides a stable position of the lens close to the nodal point and minimizes the effect of aniseikonia. However, there are situations, when zonular weakness or damage to the posterior capsule can prevent or significantly hinder this manoeuvre. The aim of this article is the presentation of modern techniques in secondary intraocular lens implantation.
