Summary: Glaucoma is a progressive, chronic disease leading to irreversible visual field loss. Vision is most threatened in patients with severe neuropathy and rapid progression rate. These patients need intensive treatment, that usually requires more than one medication. One of the most important problems in glaucoma pharmacotherapy is non-compliance. Patients who need multiple medications are poorer adherers than those on monotherapy. The authors describe factors influencing the compliance and methods that could increase adherence with glaucoma medications. Fixed combinations represent an effective and convenient solution for patients who need adjunctive therapy. They offer good efficacy, reduced exposure to BAK and often better tolerability than single components, offer better convenience and, therefore, compliance. Patients switching to a fixed combination regimen from multiple medications find the fixed combination easier to use and are more likely to continue therapy, that increases both the success chances of the therapy and the quality of life.
