Laser Therapy in Secondary Capsular and Pigmentary Glaucoma

Summary: Authors present modern methods of laser treatment for pseudoexfoliation and pigmentary glaucoma. Argon laser trabeculoplasty is effective in intraocular pressure reduction with minimal side effects. IOP reduction can be obtained in approximately 70% and maintained in about 50% for several years. In selective laser trabeculoplasty no thermal damage to the trabecular meshwork architecture occurs. It gives the opportunity to repeat this procedure. SLT appears to be an effective method also in patients treated previously with ALT. The anterior chamber morphology and angle pigmentation in pigmentary and pseudoexfoliation glaucoma seems to be perfect for laser trabeculoplasty. This method allows to reduce the amount of used antiglaucoma drugs and also to lower the total cost of glaucoma therapy. This factors improve quality of life. Laser peripheral iridotomy still remain a treatment of choice in pupillary block. In pigmentary glaucoma laser iridotomy can eliminate the reverse puppillary block and make the iris move away from the zonules. Cyclophothocoagulation is indicated for cases where other forms of therapy have failed.
