Summary: Rapidly evolving ocular technologies used during ophthalmic surgery give great opportunity of performing more advanced surgeries on older patients with more severe medical conditions. To make the surgery safe there is a need of close cooperation between ophthalmic surgeon and anesthesiologist. Despite of type of anesthesia used monitoring of intraocular pressure (IOP) remains one of the most important issues during intraocular surgery. Stable IOP is critical for the safety of intraocular procedure and must be kept within certain limits independently on patient’s life parameters during the whole surgery.
Different factors like blood pressure, venous pressure, blood level of CO2, type of anesthesia and drugs used during anesthesia may play an important role in keeping IOP stable. The aim of the article is to summarize the most important factors influencing IOP during local and general anesthesia.

Keywords: intraocular pressure, retrobulbar anesthesia, peribulbar anesthesia, general anesthesia.
