Vascular Base for Development of Circulation Alteration in Glaucoma

Summary: In this review current knowledge on vascular base for glaucoma and risk factors causing eyeball circulation disturbances occurrence has been presented. Etiology of glaucoma still remains multifactorial. Beside age, gender, race, myopia, genetics, lipid and glucose balance dysregulation, vascular risk factors have special place in pathogenesis of that disorder, among others intraocular pressure, vascular dysregulation, off-balance arterial pressure values and low ocular perfusion pressure. Additional risk factors mentioned in literature are causes for constitutional circulation and vascular dysregulation. Among them, both hypotonia and hypertonia comorbid with congenital predisposition for vasospasm occurrence are essential. In this article, functional activity of autonomic nervous, hormonal, muscular and metabolic systems which, next to endothelial cells, are the main regulators of smooth muscles in capillaries’ wall, being the basic mediator of local control in vascular resistance was written.
Conclusions: Multifactorial glaucoma etiology including compound pathomechanism creating eyeball circulation alterations, currently requires the use of many modern examination techniques. They evaluate not only the character of this pathogenetic phenomena laying at the base of glaucoma neuropathy but also assess changes in vascular blood flow, which is included in this compound process.
