Serum Anti-retinal and Anti-endothelial Cell Antibodies in Patients

Summary: Purpose: To analyze the prevalence of serum anti-retinal (ARA) and anti-endothelial cell antibodies (AECA) in patients with exudative age-related macular degeneration (AMD) in order to determine their relationship with the clinical features of the disease.
Methods: The study involved sixty-five patients with exudative AMD. Serum ARA were determined by indirect immunofluorescence method (IIF) on tissue section of normal monkey retina as a substrate and FITC – labelled polyvalent goat anti-human IgAGM serum (Euroimmun-Germany) as secondary antibody. In subjects having autoantibody with retinal vessels, the anti-endothelial cell antibodies (AECA) were detected using IIF on tissue section of monkey muscles iliopsoas and cultivated human umbilical vein endothelial cells, HUVEC (Euroimmun, Germany). Similar tests were carried out on healthy, sex and age-matched 30 controls’ sera.
Results: In all patients serum ARA were detected in titres from 1:40 to 1:1280. Their sera were found to have a significantly (p < 0.01) higher titre of ARA as compared with controls (1: 10 to 1: 40). IIF test showed the reaction within the photoreceptor layer (80%), cytoplasm of nuclear layers (24.6%), nuclei of nuclear layers (30.7%), neural fibers layer (55.5%), ganglion cells layer (32.3%) and retinal vessels (32.3%). In 21 (32.2%) sera with a positive reaction within retinal vessels on normal monkey retina, IIF test was performed on skeletal muscle and HUVEC and confirmed the presence of AECA.
Conclusions: The results of our study show that serum ARA may be an immune marker of AMD activity. To our knowledge the presence of AECA in sera of AMD patients has not been reported previously. Because it has been postulated that AMD is an immune-mediated disease, it could not be excluded that ARA and AECA may be involved in its’ pathogenesis and progression. However the precise determination of a role of circulating ARA and AECA in AMD patients requires further investigations.
