Retinal Nerve Fiber Layer Thickness Following Optic Neuritis Measured Using SOCT

Summary: Purpose: To assess the retinal nerve fiber layer (RNFL) thickness around the optic disc measured using spectral optical coherence tomography (SOCT) in patients with idiopathic optic neuritis (ION).
Material and Methods: Twelve patients with non-demyelinating ION and 11 healthy subjects at the similar age were studied using SOCT. The mean time since the onset of an acute ION was 95 months. The study participants were assigned to 3 groups. Group 1 comprised 14 eyes affected by ION, group 2 consisted of 9 fellow eyes of patients with unilateral ION and group 3 was formed by 21 eyes of healthy subjects (control group). RNFL thickness was measured across entire peripapillary area, in the superior/inferior hemispheres, in 4 quadrants and within 12 clock-hours range. For the purpose of statistical analysis the Mann-Whitney nonparametric test was used, a value of P<0.05 was considered statistically significant.
Results: In the group 1, the mean overall RNFL thickness around the optic disc, across hemispheres and quadrant segments, and in the clock hour sector 1, 2, 5, 7-11 was statistically significantly reduced when compared with the control group. In the group 2, the average total RNFL thickness as well within inferior hemisphere, temporal quadrant and in the clock hour 7, 8, 10, and 11 was statistically significantly thinner than in the corresponding sectors of the control group. Highly significant differences in RNFL thickness between group 1 and 2 was found in the clock hour 5.
Conclusion: SOCT allows detailed assessment of peripapillary RNFL thinning in patients with ION. When using a precise clock hour analysis, the statistically significant reduction in RNFL thickness across the temporosuperior, temporoinferior, nasosuperior quadrants and in the clock hour 5 was found. SOCT is also capable for detecting RNFL loss in the fellow eyes of patients with unilateral ION that are otherwise clinically and electrophysiologically asymptomatic.
