Summary: Purpose: Evaluation of retinal macular thickness using optical coherence tomography in pediatric patients with primary IOL implantation following congenital and developmental cataract surgery.
Material and Methods: 96 eyes of 62 patients (39 boys, 23 girls) at the age from 3 to 17 years (mean 9.1±3.1) were examined. Mean age at surgery was 5.7 years (±3.7), mean follow-up 6.5 years (±2.2). Group I comprised 34 children with bilateral IOL, group II 28 patients with unilateral IOL, group IV – 16 children (surgery below the age of 2 years). Control group IIa – 28 unoperated eyes from group II, group III consisted of 44 eyes of 30 healthy children (14 girls, 16 boys) at the age from 4.5 to 15.5 years (mean 9.3±2.6). CDVA, CNVA (Snellen) and AL were checked in all participants. Macular retinal thickness was examined with a scan length of 6 mm using the Fast Macular Thickness Map protocol. Mean retinal thickness at foveola (Fmin), fovea (Fov), foveal volume (Fvol), and total macular volume (Tvol) were measured with ZEISS OCT3 Stratus. The results were analyzed statistically; p(α)=0.05.
Results: CDVA values in groups I, II and IV were 0.7 (±0.2); 0.5 (±0.4) and 0.5 (±0.3), respectively. CNVA was D 0.6 (±0.3) – group I, D 1.5 (±1.0) – group II, D 1.4 (±1.3) – group IV. Mean AL was 22.6 mm (±1.4) – group I, 22.5 mm (±1.3) – group II, and group IV 22.3 mm (±1.2). Control group IIa and III CDVA was 1.0 and CNVA 0.5. Mean AL 22.6 mm (±0.7) and 22.6 mm (±0.7) in group IIa and III, respectively.
The following mean values were calculated based on OCT measurements: group I – Fmin 170 μm (±31), Fov 191 μm (±26), Fvol 0,15 mm3 (±0,02), Tvol 6,57 mm3 (±0,60); group II – Fmin 193 μm (±28), Fov 212 μm (±24), Fvol 0,17 mm3 (±0.02), Tvol 6,83 mm3 (±0.41); group IV – Fmin 179 μm (±40), Fov 193 μm (±33), Fvol 0.16 mm3 (±0,02), Tvol 6.42 mm3 (±0,77); group IIa – Fmin 163 μm (±22), Fov 192 μm (±22), Fvol 0.15 mm3 (±0.01), Tvol 6.78 mm3 (±0.29); group III – Fmin 161 μm (±23), Fov 188 μm (±20), Fvol 0.15 mm3 (±0.02), Tvol 6.62 mm3 (±0.36).
Mean retinal thickness at fovea was significantly increased in group II (p=0.001). The remaining parameters did not differ significantly between groups.
Conclusions: Measurements of retinal macular thickness in pediatric patients with bilateral IOL implantation following congenital and developmental cataract surgery were comparable to those of healthy children. The increase in mean retinal thickness at fovea found in children with unilateral IOL implantations requires further observation.
