Choroba Coatsa o nietypowym przebiegu u 12-letniego pacjenta

Coats’ Disease of Atypical Course in 12-year Old Male Patient

Summary: Introduction: Coats’ disease is an idiopathic condition characterized by abnormal telangiectatic retinal vessels and exudative retinopathy. Applied methods of treatment are: laser photocoagulation, cryotherapy and surgical retinal detachment repair. These treatments have not been very successful in achieving anatomic improvement or establishing good visual acuity.
Case Report: We present case of 12-years old boy, who came to the Department of Ophthalmology UMC in Gdansk, with visual acuity reduction in the right eye. The ophthalmological examination showed macular oedema with stella shape exudates and, in the periphery, haemangioma surrounded by intraretinal hermorrhages and elevation of the retina. Applied pharmacological treatment improved visual acuity. Performed additional examinations (fluorescein angiography, optical coherence tomography, magnetic resonance imaging, ultrasonography) allowed establishing the diagnosis of Coats’ disease. After four months significant pathological changes progression in right eye was found. Transpupillary thermotherapy of pathological retinal vessels in the right eye was performed. One month after this treatment we noticed pathologies reduction in the fundus of right eye. The patient stays under further ophthalmological observation.
Conclusions:
1. Patients with Coats’ disease need to be under regular ophthalmological monitoring because the progression of the disease could be rapid.
2. Fast applied treatment gives a chance to significantly improve visual acuity.
3. Positive results of new therapeutic methods gives a hope for improvement effects of treatment of Coats’ disease in the near future.
