How to Validate whether We Treat Glaucoma Efficiently? Review of the Contemporary Glaucoma Progression Rate Evaluation Methods

Summary: Glaucoma, according to published WHO reports, is the first cause of irreversible blindness in adults worldwide and second in developed countries. The disease is responsible for 5.2 mln of total visual loss cases (15% of all blind people population). It is estimated that in Poland glaucoma may be present even in 800 thousand of inhabitants. During the last decade there are several new diagnostic instruments becoming more widespread, possessing high sensitivity and specificity in glaucoma diagnostics. They are computer assisted and usually utilize laser scanning techniques. However, the key question after diagnosing glaucoma still remains to be answered: do we, as ophthalmologists, manage this disease efficiently enough in a long term perspective. There is a plenty of published data coming from numerous multi-center clinical trials confirming that only monitoring the intraocular pressure levels is not sufficient to measure the therapy outcomes. The key is in follow-up of retinal ganglion cell status and glaucoma progression rate measurements, performed by automated comparison analyses and careful evaluation of pathological changes appearing in time. In general, these changes can be estimated by event or trend analyses, calculated by tools specially developed and implemented in software. In the course of patient’s follow-up ophthalmologist should always require the examination results coming with comparison to the previous tests to be sure about the possible progression and its rate. This provides the correct choice of therapeutic approach, both pharmacological and surgical.
