Intereye Asymmetry of Circumpapillary Retinal Nerve Fiber Layer Thickness and Optic Nerve Head Parameters in Patients with Open Angle Glaucoma in Spectral Domain Optical Coherence Tomography

Summary: Purpose:To analyze disturbances of intereye asymmetry within optic nerve head parameters and circumpapillary retinal fiber nerve layer thickness related to primary open angle glaucoma development.
Material and methods: 187 patients were enrolled to the study and divided in four groups: normal (75 patients), primary open angle glaucoma (44 patients), glaucoma suspect (48 patients) and group where glaucoma was diagnosed in one eye (20 eyes). In both eyes of each patient the measurements of optic nerve head parameters and circumpapillary retinal fiber nerve layer thickness were performed using spectral domain optical coherence tomography. Average values from right and left eyes were compared. The absolute difference between eyes were calculated and compared with normal population.
Results:There were no statistically significant differences between right and left eyes in optic nerve head parameters except rim volume which was higher in right eyes of normal patients. The circumpapillary retinal fiber nerve layer was 4.23 µm thicker in right eyes but this difference observed in normal eyes disappeared in glaucoma and glaucoma suspect patients. The absolute difference between eyes within optic nerve head parameters and circumpapillary retinal fiber nerve layer thickness increased in primary open angle glaucoma group however statistical significance was observed only for cup volume and mean circumpapillary retinal fiber nerve layer mean value from all quadrants.
Conclusions: In normal population the circumpapillary retinal fiber nerve layer is thicker in right eyes. This right eye predominance decreases with the first symptoms of glaucoma. Advanced glaucoma is related to increased intereye asymmetry especially within cup volume and circumpapillary retinal fiber nerve layer thickness.
