The Role of Pattern Electroretinogram and Photopic Negative Response in Difficult Cases of Glaucoma Diagnosis and Treatment Monitoring

Summary: Purpose: To highlight the role of pattern electroretinogram and photopic negative response in diagnostically difficult cases of glaucoma and ocular hypertension and to demonstrate the effectiveness of these tests in monitoring the treatment of the above mentioned diseases. Material and methods: data from the literature published in PubMed between 1998–2014 and our own experience. Results: Both electrophysiological tests pattern electroretinogram and photopic negative response apply in the diagnosis and monitoring of treatment of patients with glaucoma and ocular hypertension. Discussion:The first indication of damage to the retinal ganglion cells is the result of a change in the pattern electroretinogram and, in some cases, the photopic negative response. Abnormal results in these tests are present before changes in routine diagnostic tests for glaucoma occurs (intraocular pressure, static perimetry, optical coherence tomography, nerve fiber analyzer). Conclusions: in diagnostically difficult cases of glaucoma and ocular hypertension following electrophysiological tests are advised to be performed including pattern electroretinogram and/or photopic negative response.
