Toric Introcular Lens for Astigmatism Correction in Cataract Patients

Summary: Cataract surgery is a procedure that improves visual acuity and thus raising patients’ quality of life. At the moment, patients’ expectations regarding postoperative visual quality are very high and assume the opportunity to completely get rid of the need for correction for distance.
Academic sources indicate that 30% of patients presenting for cataract surgery showed more than 0.75 D of astigmatism, 22% of patients above 1.5 D and in 8% stated above 2.0 D astigmatism.
Corneal astigmatism can be corrected after cataract surgery with spectacles, toric contact lenses or using additional surgical procedures such as radial keratotomy, astigmatic keratotomy, relaxing incisions, refractive laser treatments (Photorefractive keratectomy, LAser in Situ Keratomileusis, Laser Epithelial Kertomileusis).
The only way to remove a cataract with simultaneous concomitant correction of the refractive astigmatism is toric intraocular lenses implantation. Their implantation does not require additional surgical tools to perform additional incisions in the cornea and limbus, it is reversible and provides a predictable outcome.
This review discusses commercially available toric intraocular lenses and based on the available literature to assess their impact on patients refractive astigmatism.
Implantation of the toric lens in patients with preoperative astigmatism above 0.75 D is associated with better post-operative results compared with the refractive outcomes after monofocal intraocular lens implantation. The vast majority of patients (70–100%) achieved the best uncorrected visual acuity better than 5/10, are spectacle-independent (60–85%) and have a lower absolute residual astigmatism.
Cataract surgery with toric intraocular lenses implantation is considered to be safe procedure, giving predictable results. The effect of treatment largely depends on accurate patient selection and stability of the lens.
