Summary: Purpose: To investigate efficacy of transscleral diode laser cyclophotocoagulation and possibile complications in patients suffering from advanced glaucoma with good visual acuity. Material and Methods: Retrospective analysis of the results of treatment of 37 patients with advanced glaucoma and visual acuity at least 0.4, who underwent cyclophotocoagulation between 2011 and 2012. The follow-up was 2 years. The patients were divided into three groups depending on the type of glaucoma. Results: In all groups, the intraocular pressure decreased after two years of treatment by more than 55% compared to baseline. The visual acuity loss of ≥ 2 lines of Snellen occurred in 32.4% of patients after two years of follow-up. The number of antiglaucoma medications was reduced in 58% of patients in the first group, 81% in the second group and 71.4% in the third group. 8% of patients had complications after cyclophotocoagulation. Conclusions: Transscleral diode laser cyclophotocoagulation is a safe procedure, which can reduce intraocular pressure in eyes with advanced glaucoma. It is applied not only in glaucomatous patients with low visual acuity, but also it is an alternative for patients with advanced glaucoma and good visual acuity.
