The Role of Demodex spp. in Etiology of Eye Diseases

Summary: Demodex mites are the most common parasite found in the human skin and hair follicles. Two distinct species have been identified in human: Demodex folliculorum and Demodes brevis. Demodex folliculorum can be found in the lash follicle, Demodex brevis within Meibomian glands. Demodex folliculorum is observed in 84% of the population with mean age of 61 years and in 100% of those older than 70 years. Demodex infestation plays an important role in the occurrence of many ocular surface diseases: anterior and posterior blepharitis, Meibomian gland dysfunction, blepharoconjunctival inflammation, reccurent chalazia, corneal lesions, and basal cell carcinoma. Three mechanisms have been proposed to explain the pathogenic role of Demodex in blepharitis: direct damage, vector for microorganisms, and hypersensitivity reaction or immunological response. Ocular surface diseases caused by Demodex are still an underestimated problem with possible serious consequences for vision. Quite frequently, Demodex etiology is not considered in a differential diagnosis. Proper diagnosis and treatment in Demodex ocular surface diseases are crucial in achieving a permanent clinical success.
