The Impact of Anti-glaucoma Medications on Ocular Surface

Summary: Topical eye-drops remain the cornerstone of treatment for ocular diseases, including glaucoma. The long term use of ophthalmic medications containing preservatives, particularly when treating chronic diseases, has been linked to adverse effects and ocular surface disease. Anti-glaucoma medications decreasing intraocular pressure are often associated with ocular adverse reactions such as dry eye, and burning or stinging sensations. These undesirable effects may lead to treatment discontinuation and reduced quality of life in patients with glaucoma. Preservatives are added to multi-dose medication bottles to reduce the risk of microbial contamination. Anti-glaucoma medications usually contain benzalkonium chloride (BAK) as a preservative. Clinical studies have shown an increased incidence of adverse events with BAK and have demonstrated that the withdrawal of preservatives reduces these effects. Currently, the preservatives-free eyedrops are available, which eliminate the side effects and improve local tolerance. This article provide an overview of influence of glaucoma treatment on the ocular surface as well as the impact of known preservatives included in anti-glaucoma drug on ocular surface and quality of life.
