Endogenous Endophthalmitis as a First Manifestation of Klebsiella pneumoniae Sepsis with Pyogenic Liver Abscess

Summary: Endophthalmitis is a rare, but potentially very dangerous condition that can lead to an irreversible sight loss. In most cases it is exogenous. The endogenous form is 2–16% of described cases. The most important risk factors for endophthalmitis include HIV, AIDS, diabetes mellitus, kidney failure, intravenous drug use, urinary tract infections (including the presence of the catheter), organ abscess, previous liver transplant, cancer, infectious endocarditis and digestive system infections.
In described case endophthalmitis has developed in a patient with liver abscess and Klebsiella pneumonia bacteraemia caused by it. Despite wide spectrum systemic antibiotic therapy used, rapidly progressing eye and orbital tissue infection with vision loss was observed, which resulted in a decision to perform an enucleation. In the following observation of the patient, improvement of a general condition and absorption of the abscess have been reported over the period of a few months.
