Surgical Treatment of Pseudoexfoliation Claucoma

Summary: Pseudoexfoliation syndrome is a strongly age-related disorder in which abnormal extracellular material is produced and accumulates in intraocular and extraocular tissues. This may subsequently lead to many intraocular complications like trabecular meshwork dysfunction, phacodonesis and poor pupil dilation. Pseudoexfoliation glaucoma is considered the most common type of secondary open-angle glaucoma. Compared with primary open-angle glaucoma it has higher mean intraocular pressure, greater intraocular pressure fluctuation, more serious clinical course and worse prognosis. Therefore the treatment should be implemented at the early stage of the disease. Furthermore the patients require frequent and regular ophthalmological examination to measure intraocular pressure, observe the optic nerve and test the visual field. Surgery is recommended when medical or laser treatment are not enough to control intraocular pressure and to prevent progression of the damage caused by glaucoma. When qualifying patients to the surgery it is important to consider that due to characteristic alternations of all anterior segment tissues surgical complications are more common in patients with pseudoexfoliation glaucoma. Trabeculectomy is considered the procedure of choice in the glaucoma surgery. A growing number of studies indicate that trabeculectomy and non-penetrating filtering surgery show approximately similar results in pseudoexfoliation glaucoma and primary open-angle glaucoma in postoperative observation.
