iStents in Apen-angle Glaucoma Treatment – Literature Overview

Summary: iStents are currently the most common method of micro-invasive glaucoma surgery and the fastest growing antiglaucoma surgery in the USA. Currently two generations of iStents are used in Europe, the third generation is in phase of clinical trials. iStents are ab interno, surgical-grade devices and can be implanted alone or combined with phacoemulsification. Reduction of intraocular pressure is based on increased aqueous humour outflow through Schlemm`s canal (I/II generation) or suprachoroidal space (third generation). iStents can be used in patients with open angle, pigmentary, pseudoexfoliative glaucoma. They are dedicated to people with topical drugs intolerance or elevated intraocular pressure despite polytherapy. The level of intraocular pressure reduction ranges between 8.1–46.9% and depends on the type of iStent, number of implants, type of procedure (iStent vs. iStent + phaco), topical drugs. Recent results of randomized trials and case studies suggest that the iStents implantation is a relatively safe procedure with a low complication rate. The most common complications were stent obstruction and dislocation. Complications such as hypotension, choroidal detachment, infections were not observed.
