Diagnosis and Treatment of Preperimetric Glaucoma

Summary: Preperimetric glaucoma belongs to primary open angle glaucoma and is characterized by the optic nerve head and retinal nerve fiber layer abnormalities. However, the defects are not detected in the standard perimetry. Preperimetric glaucoma can be recognized during the first visit, if basic ophthalmological examination for glaucoma is complemented with the objective techniques (eg. Spectral Domain Optical Coherence Tomography) and the attention is paid to the retinal nerve fiber layer thickness. Despite the lack of clear guidelines, it seems advisable to start treatment immediately after diagnosis. Available work on the subject indicates that only intraocular pressure reduction ≥20% using hypotensive drops has proven beneficial effect on the inhibition of the disease progression.
