Exudative Retinal Detachment as the First Symptom of Metastatic Lung Cancer – Literature Review

Summary: The paper presents an up-to-date review of the literature concerning exudative retinal detachment accompanying neoplastic diseases available in the PUBMED database (1967–2017). The most frequent ophthalmological symptom of a disseminated neoplastic lung disease is metastases to the choroid. The signs of metastatic lesions manifesting in the eyeball may include: metamorphopsias, blurred vision, deterioration of visual acuity and pain. In routine ophthalmological examinations the metastatic focus is present in the form of a yellow or brown and grey dome-shaped lesion elevating the retina, which may be accompanied by exudative retinal detachment. The literature describes rare cases of exudative retinal detachment as the first symptom of disseminated neoplastic disease without visible metastatic lesions in the eyeball. In differential diagnostics the following examinations are utilized: ultrasonography, fluorescein angiography, optical coherence tomography, as well as laboratory and image tests of other organs (chest, abdominal cavity, brain). The treatment of metastases to the choroid involves local radiotherapy and is based on the treatment of the underlying condition with the use of chemotherapy, hormone therapy, immunotherapy, and radiotherapy. Exudative retinal detachment is treated with photodynamic and laser therapy as well as intravitreal anti vascular endothelial growth factor agents.
