Traumatic Optic Neuropathy in a Pediatric Patient with Inferior Orbital Wall Fracture – Case Report

Summary: Introduction: Presentation of a case with an unusual course – among patients with inferior orbital wall fracture, eye movement disorders and hypotropia are the most common complications. Described patient presented traumatic optic neuropathy.
Case report: A 7 years old girl got hit in the left eye area by the steering wheel of a scooter. Due to the presence of general symptoms, she was hospitalised in a children’s hospital (pediatric surgery department). Head computed tomography visualised multiple bone fractures of left maxillary sinus and left eye socket. Ophthalmological consultation (one week after trauma), showed correct mobility of the eyeball, relative afferent pupillary defect, and very low visual acuity (0.04). Visual evoked potentials (VEP) test revealed left optic nerve dysfunction. MRI examination showed no abnormalities in the area of ​​the optic canal and the nerve II itself. The patient was diagnosed with traumatic optic neuropathy. A consultation of neurosurgeons and ophthalmologists was held, with decision not to perform surgical nor conservative treatment. Follow – up consultations showed left eye vision improvement.
Conclusions: Traumatic optic neuropathy (TON) should be always considered in differential diagnosis among patients with decreased vision suffering from head trauma. TON is a therapeutic problem, because there are no standards of treatment.
