Some Practical Aspects of Intraocular Pressure Control in Patients with Glaucoma

Summary: In a review some practical aspects of intraocular pressure (IOP) control regarding mainly IOP diurnal curve and peak of IOP were emphasized. Glaucoma is a 24-hour disease with diurnal intraocular pressure (IOP) variation and IOP peaks. There is poor correlation between single measurement and the IOP peak, which may play a role of ”silent killer”, responsible for further glaucoma damage because often can not be detected during office hours. In glaucoma patients with advanced disease or with progression that is non-adequate to known IOP measurements, 24-hour IOP monitoring may suggest a greater role of IOP-related risk for glaucoma progression than previously suspected and thus may justify a more aggressive IOP – lowering treatment strategy. The follow-up of glaucoma patients should be scheduled, keeping in mind the baseline circadian rhythm and taking into account the possible timing of peak of IOP.
