Cases of complications during complex capsulorhexis

Summary: Objective: 2 cases of patients (3 eyes) with complicated capsulorhexis presentation. Material: Case 1: 63 years old female patient, who underwent surgery on thyroid gland (thyroidectomia) in March 2003. In December 2003 patient developed tetany (proper treatment was introduced). In February 2004 during examination we found mature cataract in left eye. In following two weeks patient developed intumescent cataract of that eye. We decided to perform cataract surgery. During capsulorhexis tear of anterior capsule leading to equator on both oposit sides occured. After one month the same situation repeated in the other eye. Case 2: 55 years old male patient, who suffered electric shock in left ciliary and upper palpebral region that led to tissues necrosis 12 years ago. Reconstruction was then performed. Currently patient developed cataract with anterior capsule calcification. Results: In both cases, in all eyes we managed to implant IOL?s into the capsule bag. In three months observation period we did not observe lenses dislocation. Conclusion: Despite intraoperational dificulties we should try to perform phacoemulsification and implant IOL (if it is possible) into capsule bag.
