Complications after Transpupillary Thermotherapy Used in Intraocular Tumors Treatment

Summary: Transpupillary thermotherapy (TTT) is a method for treating small melanomas. An infrared diode laser (810 nm) and a modified slit-lamp delivery system deliver energy through the pupil to the choroid. This causes elevation in local tissue temperatures to 45-60°C resulting in tissue necrosis. TTT is capable of causing necrosis of tumors up to 3.9 mm in thickness. To reduce the risk of recurrence “sandwich” treatment can be used where TTT treats the tumor apex and brachytherapy treats the tumor base. This combination therapy enables TTT to be used in tumors greater that 4 mm in thickness and may decrease the risk of radiation related complications.
The most common complications associated with TTT include: iris lesions, irydocyclitis, posterior synechiae, cataract, haemorrhages within the tumor mass, subretinal and intravitreal haemorrhages, overlying branch vascular occlusion, macular edema and preretinal fibrosis, optic disc edema and retinal detachment.
