Summary: Purpose: Objective and subjective assessment of the impact of the treatment on a drip surface of the eye in patients treated chronically with primary open angle glaucoma.
Material and methods: The study included 90 patients – 180 eyes (60 people diagnosed with primary open angle glaucoma and 30 during the diagnostic process). The study group consisted of 60 patients treated with monotherapy – prostaglandin analogues: 30 people used a formulation containing preservative – benzalkonium chloride (latanoprost), 30 people preservative-free formulation (tafluprost). The control group consisted of 30 patients not using any drops. The study evaluated: intraocular pressure (Goldmann applanation tonometry), Schirmer’s test with topical anaesthesia, ocular surface staining with fluorescein without preservative use (conjunctiva and cornea) with the Oxford scale index, the Tear Break Up Time, the degree of ocular surface irritation (M.C. Monnies scale), Ocular Surface Disease Index and level of extracellular matrix metalloproteinase MMP-9 in the RPS technology – strip test InflammaDry.
Results: Positive result for the presence of MMP-9 was measured in 46.7% of patients using the drug with a preservative. In other groups positive for MMP-9 received 16.7% of patients. The difference between examined groups was statistically significant. The Tear Break Up Time <5 occurred in 50% of patients using formulation with preservative, in the other groups only in 10%.
Conclusions: The study results confirm the fact that the formulation without preservative has positive effect on the ocular surface condition, while providing full control of intraocular pressure. It should be emphasized that in 46.7% patients treated with a preservative formulation objective signs of inflammation of ocular surface have been obtained in a simple test with very good specificity and sensitivity.
