Summary: Flaps most frequently used in eyelid surgery are described as local, meaning that they are taken from a neighboring area. Flaps can be classified as sliping, advancement, rotation, or transposition. Flaps may also be classified according to their blood supply as either random or axial. The random flap receives its blond supply from segmental, anastomotic arteries which lie deep below the muscle layer under the flap. These vessels send perpendicular, perforating musculo-cutaneous arteries to the interconnecting derma plexae of the flap.
The axial flap receives its blond supply through a direct cutaneous artery, which lies above the muscle layer in the flap. The autors present own experience in using local flaps in eyelid surgery.
