Bilateral Serous Retinal Detachment in Pregnancy Complicated with Preeclampsia – Case Report

Summary: Introduction: Preeclampsia is a disorder that occurs after the 20th week of pregnancy, during delivery or in the puerperium period. It is characterised by blood pressure increase (≥140/90 mmHg as measured twice at a six-hour interval) and proteinuria (≥300 mg per day). Severe forms of preeclampsia may be accompanied by various types of ocular complications, amongst others, SRD (serous retinal detachment).
Aims: The aim of the study is to present a preeclamptic woman with bilateral SRD and the assessment of the impact of the disease on the final visual acuity.
Material: Patient, 28 in the 37th week of pregnancy, admitted to hospital with severe headaches, severe vision deterioration, and raised blood pressure. The patient was diagnosed with preeclampsia complicated by haemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome. After a caesarean section performed in an obstetric emergency, the patient developed preeclampsia, acute renal failure and bilateral serous retinal detachment.
Methods: The observation period lasted 12 weeks. A full eye exam was performed, ocular ultrasound, fluorescein angiography, and optical coherence tomography. The patient received pharmacological treatment.
Results: The final visual acuity was 1.0 in both eyes.
Conclusions: The bilateral serous retinal detachment in the preeclamptic patient continued its course with the idiopathic resorption of subretinal fluid within several weeks. The early diagnosis and casual conservative treatment led to a complete reattachment of the retina and complete recovery of visual acuity in both eyes.
