Safety and Efficacy of the Viscocanalostomy in the Surgical Management of Primary Open Angle Glaucoma

Summary: Purpose: To assess the pressure-lowering effect of the viscocanalostomy – nonpenetrating filtering procedure.
Methods: Thirty-seven eyes with medically uncontrolled primary open-angle glaucoma underwent viscocanalostomy procedure. After a superficial scleral flap was raised, a deep sclerectomy was performed in the scleral bed, deroofing of Schlemm`s canal and preparation of a window of Descemet`s membrane. Examinations were performed before surgery and 1 day and 1, 3, 6–9 and 12 months postoperatively.
Results: Mean preoperative intraocular pressure (IOP) was 19.46 mmHg ± 3.14 (SD). Mean postoperative IOP was 14.13 ± 5.32 mmHg at 1 day, 14.65 ± 3.04 mmHg at 1 month, 13.52 ± 2.81 mmHg at 3 months, 14.52 ± 2.73 mmHg at 6–9 months and 15.19 ± 2.54 mmHg after 1 year. The following postoperative complications occured: hyphema (2%); postoperative hypotony less than 10 mmHg (19%) after 1 day and 0% after 1 year.
Conclusion: The results reported here show that viscocanalostomy is safe and effective surgical procedure in the management of primary open angle glaucoma with fewer complications than the current literature reports for trabeculectomy.
