Pellucid Marginal Corneal Degeneration – Literature Review

Summary: Marginal pellucid corneal degeneration is a congenital ectatic disease of the cornea, in which a defect of the metabolism of collagen fibrils seems to play the key role. The degeneration is characterized by a semicircular area of corneal thinning which, in extreme cases, reaches only 20% of the thickness of normal tissue. Thinning band width of about 1–2 mm extends approximately 1–2 mm from the limbus, being separated from it by unchanged tissue. Within the thinning, disorders such as vascularization, lipid deposits or epithelial defects are never to be observed. Currently, it is postulated that the pellucid marginal degeneration jointly with other ectatic corneal diseases are different clinical courses of a single disease.
The aim of this paper is to present the problem based on literature data on pellucid marginal corneal degeneration, taking into account the clinical course of the disease and its possible complications, as well as diagnostic problems and treatment options available. In the area of invasive treatment special emphasis was put on the procedure of corneal collagen cross-linking, which is considered to be a key component of the treatment procedure enabling the inhibition of ectatic progression.
