Summary: Congenital lacrimal duct obstruction appears in about 20% of children in the first months of life and presents with epiphora, tear lake and/or mucopurulent discharge in conjunctival sac. In the majority of patients spontaneous resolution is observed during the first year of life, conservative treatment including hydrostatic lacrimal sac massage and topical antibiotics if indicated are sufficient. After conservative treatment has failed, probing of nasolacrimal duct is performed and then, if unsuccessful, intubation is recommended. Both methods have high success rates in pediatric patients but in case of persistent symptoms, the gold standard of definitive treatment is dacryocystorhinostomy.
