Summary: The most common causes of pediatric orbital fractures are sports injuries and traffic accidents. In the majority of cases rapid and proper management enables complete and permanent healing. However, coexisting damage to extraocular muscles requires a comprehensive cooperation between ophthalmologist, radiologist and maxillofacial surgeon. In some cases, radiological findings can be misinterpreted, giving a false impression of a direct extraocular muscle rupture. In such cases it is crucial to remember that in case of isolated orbital floor fractures which are not associated with the penetrating orbital trauma, a rupture of the inferior rectus muscle is very uncommon. This paper presents a case in which proper interpretation of signs and symptoms confronted with radiographic images led to an adequate management.
